The core difference
A hair transplant is surgery. Follicles are taken from a donor area (usually the back of the head, which is genetically resistant to pattern loss) and implanted into thinning or bald areas. Once healed, transplanted hair generally continues to grow because it keeps its donor characteristics.
Exosome therapy is non-surgical. It aims to improve the health of follicles that are miniaturising but still present. It does not add follicles and cannot cover a completely bald area.
